Enrollment Agreement
REFUND / CANCELLATION POLICY
The Student has the right to cancel this enrollment agreement at any time prior to the completion of 60% of the courses enrolled (whether or not Student actually attends the class in person or online), which is determined as either (collectively, the “Cancellation Deadlines”):
The completion of the 6th class (Single Course)
The completion of the 12th class (Course Bundle)
A Notice of Cancellation must be submitted to IO Music Academy in writing. Afterwards, sStudent shall receive one of the following:
Full Refund. Students that submit a Notice of Cancellation before the first day of instruction will receive a 100% refund of the Course Fee (excluding the non-refundable Registration Fee of $250).
Pro-Rata Refund. Between the first day of instruction up until the applicable “Cancellation Deadlines” above, refunds of the Tuition (excluding the non-refundable Registration Fee) will be issued on a pro-rata basis based on the number of classes completed at the time the Academy receives Student’s Notice of Cancellation.
Students are not eligible to receive any refund for any notice of cancellation received by the Academy after the Cancellation Deadline. The pro-rata refund will be calculated based on the number of classes completed divided by the number of total classes enrolled.
ACCIDENT WAIVER & RELEASE; INDEMNITY
Student understands and acknowledges that there may be small risks of personal injury and property damage while participating in activities and using the equipment at the Academy (the “Facilities”). With full understanding of the risks involved and despite this Release, Student hereby agrees to assume all of the risks and responsibilities in any way associated with the Facilities and activities offered by the Academy.
In consideration of and in return for the services, Facilities, and other assistance, provided by the Academy and its instructors, Student hereby waives all claims against, and releases the Academy, its agents, and its instructors from any and all liability, claims and actions that may arise from injury or harm to Student, or from damage to Student’s property in connection with their use of the Facilities. This Release covers claims caused entirely or in part by any acts or failures to act by the Academy, including but not limited to negligence, mistake, or failure to supervise; but will not apply to claims arising from the Academy’s gross negligence or willful misconduct, Student also agrees to defend, indemnify, and hold the Academy harmless from all third-party claims for any personal injury, death, or property damage caused entirely (or in part) by Student’s conduct.
MEDIA RELEASE
Student acknowledges and agrees that the classes will be recorded and that their likeness may be captured in those recordings. In addition, the Academy may take photographs, videos, or digital recordings of Student and may use those images for marketing purposes. Student hereby consents to the Academy’s use of their likeness, image, name, and identity in those marketing and promotional materials or the dissemination of any class videos that may show Student’s image or likeness. Student agrees that there will be no compensation for the Academy’s use of Student’s likeness in any such marketing materials or class recordings.
SECURITY & SURVEILLANCE
Student acknowledges that in order to uphold the security of IO Music Academy, there are cameras located throughout the building. Cameras will not be actively monitored but are accessible to a limited number of trusted individuals and will be referenced if deemed necessary to uphold the safety and security of IO Music Academy.
